Barneyis the hospital janitor — a lore NPC rather than a threat or a playable class. He has worked at the hospital longer than anyone, roams the hallways and maintenance areas, and volunteers cryptic warnings to players who slow down long enough to listen. His dialogue is where a large part of the hospital's backstory lives.

5. Characters vs Playable Classes
Why the same names show up twice
Several names on this page are both a character in the world and a class you can play. Nurse, Head Nurse, Secretary, Security, Paramedic, Surgeon, Psychologist and Secret Agent all appear as staff roles, and each also exists as a purchasable class with its own perk, cost and levels.
Keeping those two ideas separate prevents a lot of confusion. Reading about the Nurse as a character tells you who works the front desk; reading about the Nurse class tells you what 10 Animal Coins actually buys. As a rule: when a page discusses perks, costs and levels it is describing the class, and when it discusses location, quests and dialogue it is describing the character.
Barney is the clearest exception worth remembering — he is a character only. There is no Barney class, no Barney perk, and no amount of Animal Coins that lets you play as him.
