Intern vs. Your First Real Class
The Intern climbs from +10 to +20 bonus sanity across its three levels, and that is the entire kit. It never gains inventory space, never starts a shift holding equipment, and never converts your work into extra sanity — every other class in the game does at least one of those things. The table below lines the Intern up against the two upgrades most players actually buy first.

Read that last column carefully. Nurse turns ten Animal Coins into permanent carry capacity, and Surgeon turns every patient you heal into a rolling sanity engine. The Intern gives you a head start that you spend once and never get back.

Skins are cosmetic only. None of them change perks or sanity numbers, so there is never a gameplay reason to chase one. Limited-time exclusives cannot be obtained again once their window closes, which is why the notes below matter only for collectors.

How to Play Intern Efficiently
Treat the bonus sanity as a learning budget rather than a resource to protect. Because the Intern has no tool or inventory advantage, the only thing that improves your result is how quickly you learn to spot anomalies — so spend your first shifts deliberately:
- Anchor on the patients you have seen before. Compare what is in front of you against the earliest version of the room you can remember; most anomalies are caught by noticing what changed, not by memorising a list.
- Do not hoard the buffer. The extra sanity exists so that you can afford mistakes while learning. A shift where you never risk anything is a shift where you learned nothing.
- Save toward Nurse before anything else. At ten Animal Coins it is reachable within a couple of shifts, and it converts to permanent inventory immediately.
When to Upgrade
The Intern class is a tutorial tool. It gives bonus starting sanity to help you learn the basics, but it provides no meaningful combat, utility, or inventory benefits.
Upgrade as soon as you can afford:
- Nurse (10 Animal Coins) — Best first upgrade. The extra inventory slots are game-changing.
