Who Is the Masked Figure?
The Masked Figure is a rare, hostile entity that appears in the hallways of Animal Hospital during late shifts (Shift 7+). It wears a featureless white mask and a dark coat, and it does not speak or make sounds. Its presence alone causes rapid sanity drain, and it pursues players relentlessly until line of sight is broken.
Unlike the Ghost (which spawns after patient death) or Skinwalkers (which enter from failed check-ins), the Masked Figure appears without warning and follows no predictable spawn pattern. Its connection to the hospital lore is still unknown, though some players theorize it is related to the Animal Corp experiments mentioned in Liz's dialogue.
Behavior & Spawn Conditions
- Only appears on Shift 7 or later.
- No known trigger — it can appear at any point during the shift.
- Moves at a slow, steady walk — but it cannot be outrun in tight corridors.
- Causes rapid sanity drain when in line of sight.
- Does not respond to Tasers or Guns (unlike Ghosts and Skinwalkers).

How to Survive
Break Line of Sight
The Masked Figure despawns after losing sight of you for a few seconds. Duck into a room and close the door immediately. Wait 5-10 seconds before peeking out.
Use Doors
It cannot open doors. Create a trail of closed doors behind you as you move through the hospital. This gives you safe zones to retreat to.
Avoid Tight Corridors
If you see it at one end of a hallway, go the other way. The CCTV cameras help — check camera feeds to locate it before entering a hallway.
Do Not Fight
Weapons do not work on the Masked Figure. Do not waste Taser charges or Gun ammo. Save them for threats you can actually eliminate.
Unkillable Threats Compared
The Masked Figure belongs to a small group of threats that weapons simply do not solve. Because all of them make you want to reach for a Taser, it is worth knowing which ones are immune before you spend charges that could have gone to something you can actually kill.
