How to Access the Basement
- Complete the Doctor's Key quest — Talk to Ratthew, investigate Room 5, solve the puzzle for the Doctor's Key. See the Doctor's Key guide and Room 5 guide.
- Get the Basement Key Card — Use the Doctor's Key to open the safe in Room 5. The Basement Key Card is inside.
- Find the basement entrance — Located on Floor 1, near the janitor closet. Requires the Basement Key Card to unlock.
- Descend carefully — The stairs are dark and narrow. Bring a flashlight and weapon.
What's in the Basement
Lower Containment
Locked cells containing high-tier anomalies. Do not open the cells unless you are prepared to fight. Contains rare loot if you survive.
Loot Lockers
Two lockers in the basement corridor and lower containment room. Contents include Teddy Bears, Coffee, and cash. High-value loot spawns here.
Lore Documents
Scattered notes and reports detailing Animal Corp's experiments. Essential for understanding the full story behind the anomaly outbreak.
Safe Room
A small secure room near the stairs. No threats spawn inside. Use it as a resting point and inventory checkpoint.
Survival Tips
- Do not go alone. Bring at least one teammate for co-op.
- Bring a flashlight and spare batteries — the basement is completely dark.
- Keep a weapon (Gun or X-Taser) ready at all times.
- Basement Key Card is lost each shift — you must re-unlock the basement every round.
- Do not open containment cells unless you are prepared to fight high-tier anomalies.

Is the Basement Worth It?
The basement is the one part of the hospital that is genuinely optional, which makes it the one place worth deciding about in advance rather than wandering into. Everything below the stairs is a trade, and the trade is roughly this:

The honest verdict is that the basement pays for itself only when the rest of the shift is already under control. Going down while patients are queuing upstairs trades a certain, immediate cost — the queue — for an uncertain reward you might not survive to spend. The players who get value out of it treat the trip as a planned objective: go early in the shift, go with a teammate, and go for a specific reason rather than out of curiosity.
It is also the section of the game most worth visiting for the story rather than the loot. If you are chasing the narrative behind the outbreak, the lore pages collect what the basement documents are building toward, and the Doctor's Key quest is the chain that leads down here in the first place.
Basement — FAQ
Where is the basement entrance?
On Floor 1, near the janitor closet. It will not open without the Basement Key Card, so finding the door before you have the card is not useful.
Do you keep the Basement Key Card between shifts?
No. The card is lost each shift, so the unlock is a repeatable part of the loop rather than a one-time achievement. Plan the Room 5 safe visit into every run where you intend to go down.
Can you survive the basement alone?
It is possible but not advisable. Darkness, high-tier anomalies, and locked cells all punish a solo run, and there is nobody to cover you if a containment cell goes wrong. Bring at least one teammate.
Should you open the containment cells?
Only when you are prepared to fight what is inside. The cells hold high-tier anomalies and the loot behind them is the reward for winning that fight, not a shortcut around it.
