He sometimes leaves useful items in his mop buckets — check them when you pass by.

Practically, that means two habits are worth forming. First, slow down when you see him: the hints are attached to proximity and patience, not to any menu or prompt. Second, treat what he says as directional rather than literal. His warning about the basement is the clearest example — it tells you something is deliberately hidden down there rather than handing you a step-by-step route.

It is worth being clear about Barney's place in a shift, because players often expect him to behave like a class perk. He does not restore sanity, carry equipment, or fight anomalies, and he will not rescue a run that is already collapsing.

What he does is supply context: lore about the hospital's history, tips that apply to surviving anomalies, and occasional appearances during key story moments. That makes him optional in the mechanical sense and valuable in the narrative sense. If your goal is simply finishing a shift with sanity to spare, Barney changes nothing. If your goal is understanding why the hospital is the way it is — and getting early warning about places like the basement — he is one of the most useful characters to talk to.
