Dr. Harlow Secret Interaction
Give Dr. Harlow a Mint Tea ($100 from the Supplies Shop) to unlock his special lore dialogue about the hospital's visitors — a hidden achievement.
Ceiling Eyes Taming
You can tame the Ceiling Eyes ceiling-dweller by using Eye Drops on it, turning a threat into a tool.
No-Weapon & Trash Tricks
Kill anomalies without weapons using environment interactions, and throw fainted (unconscious) patients into the trash to avoid penalties.

Second Floor Secrets (Lab Update)
The Second Floor expansion added several mechanics the game never explains. The entries below are condensed from BlaxieBLOX's secret run. The lore entry is kept deliberately high-level — the documents themselves are the payoff, and they are already waiting in the room.
Four DNA samples from a single anomaly
Let a monster aggro onto you while it is chasing you across the hallway, then interact with it holding the lab syringe: the game registers an active anomaly as a high-density target and hands you a full four-sample stack in one grab. Sample a mimic before it transforms and the trick stops working, because that entity is already flagged as harvested.
Maple syrup is a secret ingredient
The lab shelves show four ingredients, but maple syrup is a hidden fifth. Mix it with DNA from a patient carrying zero infection and the result is a throwable explosive rather than a serum — it detonates like a gunshot and knocks everything in the radius flat, clearing a crowded room without spending weapon ammo.
A craftable fire potion
Room fires used to be random, so the old 'let a patient walk into the flames' shortcut was unreliable. Brewing the fire recipe makes it repeatable: smash it in the reception doorway and incoming patients drop at the entrance, where they can be carried straight to the bin.
Green serums stack
Green buff serums are not one-and-done. They stack and persist for the rest of the run, which is how players reach the extreme movement speeds you see in clips — one bottle looks weak, a batch does not.
The ghost serum removes collision
Drinking the ghost serum drops normal collision with parts of the map. You can walk through closed doors and lock shutters and even slip past the outer walls, which is the shortcut into rooms that are otherwise sealed.
Hidden documents in the clown room
After the clown badge, stay in that room and pick up the newspaper the clown leaves behind. Its issue number is far higher than the paper found at Liz's house, and its front page reports wards being permanently closed plus an Animal Corp road accident. An incident log on the shelf identifies the clown as a former employee linked to the lab's first biological experiments. Neither document is referenced by any current quest.
A shortcut out of the secret room
The hidden room beneath the Lab has a second way out that drops you back near the ground-floor reception lockers, so you can move between floors without climbing the stairs every time. TechWiser's Second Floor guide documents the exit — treat it as a route time-saver rather than a required step.

What the clown-room files actually say
The shelf documents in the secret room are the real payoff for the Clown badge, so this is a structure-level summary rather than a line-by-line transcript. The precise figures are printed on the in-game files — read them yourself, because a spoken reading is easy to mishear, and we do not republish numbers we have not confirmed.
- The incident report is filed against an employee, not a patient — a long-serving hospital entertainer whose stage name is used throughout the document.
- The report sequences the incident in the laboratory area first, then an attack on the occupants of a recovery-wing room.
- It closes with the subject's whereabouts listed as unknown, and the remainder of the page is described as water damaged — the same “corporation closed its own file” shape the newspaper and the shelf log already imply.
- Everything beyond that — including the reading that this clown was the hospital's first experimental subject — is video-author interpretation, not text printed in the game. We keep it marked as a theory.
